In Rekonnect, Forms are customizable feedback tools used to collect input from users about their experience with training, documents, or product-related content. Forms help MedTech companies understand user satisfaction, identify training gaps, and collect structured data for quality improvement. They can be embedded in the user experience or sent out proactively to gather insights.
Forms
A Form is a configurable feedback component that allows users to submit opinions, ratings, or structured responses. Administrators can create forms with different question types and link them to training packages, courses, or documents. Submitted responses are collected as Received Feedback for review and action.

Using forms for feedback collection:
Forms can be used in two primary ways:
Embedded Feedback: Displayed directly within the user experience, e.g., after completing a course or viewing a document
Standalone Surveys: Sent out via email and linked to specific training packages, products, or general topics
Linking forms to content:
Forms can be linked to:

Access and visibility of forms:
Only users assigned to the relevant training package or course will see associated forms. Forms can be configured to:
Appear at the end of a course
Trigger after a document is viewed
Be available in the User Portal as a call to action
Tracking received feedback:
Once a user submits a form, it becomes a Received Feedback entry. These can be accessed by administrators and reviewed through the Admin Portal. Each response includes:
Submission time
Linked content (training, course, or document)
User input across all form elements
Versioning and editing forms:
Forms are editable and can be iterated on, but submitted feedback entries are preserved. When a form is updated, new responses reflect the updated version, while old responses remain linked to the earlier form structure.
